Hawaii Department of Education SURVEY FOR PARENTS / GUARDIANS

Your feedback about our public schools is important to us. This year, we respectfully request a few moments of your time to complete two parts of a combined survey. The first part is the annual School Quality Survey (SQS), which measures your perceptions of the safety and well-being of your child and your satisfaction and involvement/engagement with the school. The second part is the 2021 End-of-Year (EOY) survey which asks for your input on your child’s learning this year. Your responses will be kept anonymous in order to gather candid information and will be used by schools for continuous improvement purposes. As a parent/guardian, this is an opportunity for your voice to be heard.

Parents/guardians may complete one survey per child. You may complete the combined SQS and EOY Parent Survey by completing the survey online https://www.surveymonkey.com/r/2021SQSEOYParent. Schools may have a computer with internet access available to complete the surveys at the school site. If you do not have internet access, you may request a paper survey, one for each child, from your school no later than May 7, 2021.

Please read the survey directions carefully and complete the paper or online survey no later than May 28, 2021.
